After 2+ years of daily street food in Vietnam: ice is safe, tap water isn't, avoid dead restaurants, use limes on chopsticks, and trust local spots over international chains.
We've lived here for over 2 years eating street food every day and we're still standing. Ice is safe, tap water isn't. Empty restaurants at peak lunch = skip, there's a reason they're empty! Big international chains seem to consistently make us sick here, but we've never had issues with the local stalls. Raw veg is fine, and always rub your chopsticks with lime. Trust the busy spots with locals, if they weren't safe the locals would be the first to know.
